Applecross PERIODONTICS - Surgery
Access Flap Surgery
Sometimes periodontal surgery is required in areas that do not respond to initial root planing. Surgery generally involves gently lifting the gums to get better access to deep sites or hard to reach places such as furcations or root grooves. Most people don’t require surgery and certainly not for the entire mouth. Treatment plans for surgery sometimes follow the initial phase of non-surgical treatment to improve prognosis, as long as an excellent standard of oral hygiene had been achieved.
Crown Lengthening
- A resective periodontal surgical procedure requiring recountouring of the gingiva and usually the supporting bone.
General indications include
- Subgingival caries
- Perforations or fractures in the coronal third of the root
- Inadequate retention for a restoration due to a short clinical crown
Anterior indications include
- Aesthetics
- Altered passive eruption
- Excessive gingival display
- Attachment loss due to periodontal disease
Limiting factors include
- Patient health
- Anatomical considerations
- Aesthetics
- Cost
- Root dimensions
- Length & Width
- Anteriorly soft tissue support
- Prognoses of adjacent teeth following surgery
- Furcations
Alternatives may include
- Extraction
- Orthodontic extrusion with or without crown lengthening
- Compromise on aesthetics
- Orthodontics to intrude with or without crown lengthening
Surgery involves gingival contouring to desired position followed by complimentary osseous recontouring to establish biologic width at new level.
In most cases the patients are referred to Applecross Periodontics by their dentist. Generally we work as a team with your referring dentist, but a referral by a dentist is not required for a visit to our office. We welcome referrals from patients and friends of the practice as well as self-referral patients.

If you have noticed bleeding or swollen gums, bad breath or taste, sensitive or loose teeth, shrinking gums or food impaction then call us for a consultation. These are sure signs of gum disease. If you have diabetes or are a smoker then you are at a much greater risk of developing gum disease.
