GRAFTING
Mucogingival surgery was first discussed in 1957 by Friedman to include procedures aimed at preserving or increasing keratinised tissue
Nowadays the emphasis has changed and we are much more focused on replacing lost tissue and covering root surfaces that are of aesthetic or functional concern
True gingival recession will not correct itself.
In the literature there are numerous problems with measurements of recession. One paper noted that “a slight recession in a facially displaced mandibular incisor was reversed after self-alignment” (Andlin-Sobocki et al J Clin Perio 1993)
Creeping attachment “spontaneous improvements” may be associated with a similar phenomenon as creeping attachment with 0.8mm increase in attachment observed over first year after grafting. Very unlikely to have any improvement if little or no keratinised tissue exists.
Graft 6 months post-operatively
Graft 3 years post-operatively (the same patient)
